Rural Health Nurse Certification Program Enrollment 

To enroll in this Program you must complete the following two steps:

    1. Complete the information application below
    2. Upload your signed letter of support from your manager/supervisor

Upon receipt of both items listed above, enrollment documents will be reviewed, and a follow-up email will be sent to confirm admission and provide payment options.  

Please submit these two items together. Enrollment cannot be finalized without both the information form and signed letter.
